Abstract: When it comes to the care of elderly people, the use of physical restraints is a challenging and complex problem. Despite the well-known correlation between the use of physical restraints and the onset of adverse effects (physical and cognitive worsening as well as an increased mortality rate), some caregivers and healthcare providers continue to use physical restraints indiscriminately, especially within the retirement homes, where around 63% of the physical restraints are bed rails.
Abstract : El uso de contenciones fÃsicas es un problema complejo y desafiante en el cuidado de personas de edad avanzada. A pesar de la asociación de las restricciones fÃsicas con diversos efectos adversos (empeoramiento funcional y cognitivo, y aumento de la mortalidad persiste todavÃa una alta frecuencia de uso de restricciones fÃsicas por parte de cuidadores y proveedores de asistencia sanitaria especialmente en el ámbito residencial donde alrededor de un 63 % del total de las contenciones son de tipo baranda.

Introducción
El uso de contenciones físicas es un problema complejo y desafiante en el cuidado de personas de edad avanzada. A pesar de la asociación de las restricciones físicas con diversos efectos adversos (empeoramiento funcional y cognitivo, y aumento de la mortalidad persiste todavía una alta frecuencia de uso de restricciones físicas por parte de cuidadores y proveedores de asistencia sanitaria especialmente en el ámbito residencial donde alrededor de un 63 % del total de las contenciones son de tipo baranda.Objetivos
El objetivo de este estudio es resumir la historia desde los orígenes de las contenciones físicas hasta el día de hoy, analizar las instituciones sensibilizadas y que promueven el desarrollo de la calidad de vida de las personas cuidadas teniendo en cuenta la atención centrada en la persona sin sujeciones para que se garanticen los derechos y la dignidad; y por último identificar el perfil de seguridad y eficacia de los dispositivos inteligentes de contención física desarrollados en los últimos 10 años mediante una revisión bibliográfica
Material y métodos
Criterios de elegibilidad: artículos publicados en revistas indexadas (Q1, Q2, Q3, Q4), en cualquier idioma, publicados en los últimos 10 años; también se incluyeron tesis, tesinas y/o capítulos de libros que por su relevancia se consideraron relevantes de incluir.
Fuentes de información: se realizó una revisión sistemática de estudios publicados en las bases de datos PubMed, Cochrane y Google académico. Se evaluó la calidad de los estudios y se extrajo la información de los que reunieron los criterios de inclusión establecidos desde los meses de septiembre 2021 hasta enero 2022.
Estrategia de búsqueda: palabras clave con términos MeSH y otras palabras relacionadas con el tema (Elderly, Physical restraints, Nursing Homes, Bed Rails).
Proceso de extracción de los datos: se han analizado 86 artículos, 4 capítulos de libro y una Tesis Doctoral en las diferentes bases de datos mencionadas anteriormente. De todas las publicaciones analizadas, únicamente han sido seleccionados 28 artículos, 2 capítulos de libro y una Tesis, que cumplen los criterios de inclusión especificados en la revisión.
Resultados
La creación de contenciones mecánicas surgió como respuesta al problema de las enfermedades mentales. Con ellas se pretendía mantener aislado y controlado a los pacientes. Pese a su falta de eficacia y seguridad, la limitación en el uso de restricciones sigue siendo un reto difícil de cumplir las restricciones físicas todavía se aplican ampliamente, sobre todo en el ámbito residencial, donde hasta el 63.40% del total de las restricciones son sujeciones mediante barandillas en la cama y donde su uso se sigue considerando "necesario" por parte de muchos profesionales con el objetivo de garantizar la seguridad de las personas. La mayoría de las personas sometidas a restricciones físicas son precisamente personas mayores de edad avanzada que, comparado con personas más jóvenes, presentan un riesgo de caída 3 veces mayor, incluyendo las caídas de la cama. La presencia de deterioro cognitivo y la alta dependencia son factores que aumentan el riesgo de sufrir contenciones físicas.
Investigar asociaciones sensibilizadas y que promuevan el desarrollo de la calidad de vida de las personas cuidadas teniendo en cuenta la atención centrada en la persona sin sujeciones para que se garanticen los derechos y la dignidad ha sido una de las líneas de investigación de este trabajo y por último se han identificado el perfil de seguridad y eficacia de dispositivos inteligentes de contención física desarrollados en los últimos 10 años.
Conclusiones
A pesar de la alta prevalencia en el uso de contenciones y dificultades manifiestas para disminuir su aplicación, todavía es limitada la evidencia científica sobre el uso de los diferentes tipos de contenciones en diferentes personas y situaciones, menos aún sobre productos innovadores y modernos aplicables a estas personas que puedan ayudar y limitar las contenciones físicas

Introduction
When it comes to the care of elderly people, the use of physical restraints is a challenging and complex problem. Despite the well-known correlation between the use of physical restraints and the onset of adverse effects (physical and cognitive worsening as well as an increased mortality rate), some caregivers and healthcare providers continue to use physical restraints indiscriminately, especially within the retirement homes, where around 63% of the physical restraints are bed rails.
Objectives
The objective of this study is, to sum up the history of the physical restraints from their origins to the present, to analyze the institutions sensitized to this topic which, therefore, promote the quality of life of the elderly without the use of physical restrictions to ensure their rights and their dignity. Moreover, throughout a literature review, our study aims to identify the efficiency and safety of the smart devices that have been developed in the last 10 years in the field of physical restraints.
Material and methods
Eligibility criteria: Published articles in indexed journals (Q1, Q2, Q3, Q4) in any language, published in the last 10 years. Theses, dissertations and/or book chapters that we considered relevant for our study were also included.
Data sources: A systematic review of published articles in the database of PubMed, Cochrane and Google Academic was carried out. After evaluating the quality of each study, we compiled the relevant information from the studies that met the inclusion criteria. The information was gathered from September 2021 until January 2022.
Search strategy: Keywords based on MeSH (Medical Subject Headings) and other words related to the topic (elderly, physical restraints, nursing homes, bed rails).
Data mining: 86 articles, 4 book chapters and a PhD thesis from the aforementioned different databases were analyzed. From all the literature reviewed, we selected 28 articles, 2 book chapters and a thesis, which met the inclusion criteria mentioned in the revision.
Results
The emergence of physical restraints arises in the context of mental illnesses. These physical restraints aimed to isolate and control the patients. Despite their lack of efficiency and safety, limiting the use of these restraints remains, still nowadays, a challenge. They are overly used, especially in nursing homes, where up to 63.40% of the physical restraints come in the shape of bed rails. Moreover, the use of these physical restraints to guarantee the person's safety is considered "necessary" by most healthcare professionals. Most people subject to physical restraints are the elderly who, compared to young people, are 3 times more likely to fall, including falling from bed. Patients with cognitive impairment and high dependency are more likely to be subject to the usage of these restraints.
The study of entities sensitized to this issue and institutions that promote the development of the elderly without the use of physical restrictions to ensure their rights and their dignity has been one of the research areas of this study. Finally, the efficiency and safety of the smart devices that have been developed in the last 10 years in the field of physical restraints have been identified.
Conclusions
Despite the high prevalence of the use of physical restraints and the challenge that represents diminishing their use, the scientific evidence regarding the different types of restraints for different people and situations is still poor. And even more so, when it comes to innovative products applicable to these people, which could be used to help them limit the use of physical restraints.
