Abstract: During the COVID-19 pandemic, non-essential rehabilitation services were affected, including those for urinary incontinence (UI). UI can affect one in four people and associate with negative repercussions on the quality of life and the health system. In order to maintain continuous and effective care, a program of face-to-face sessions combined with telerehabilitation (Tele-RHB) was designed using pelvic floor muscle training (PFMT) as first-line treatment and evaluating adherence as a key factor.
Abstract : Durante la pandemia por COVID-19 se vieron afectados los servicios de rehabilitación no esenciales, entre ellos los de incontinencia urinaria (IU). La IU puede afectar a una de cada cuatro personas, asociarse con repercusiones negativas en calidad de vida y para el sistema sanitario. Con el fin de mantener una atención continuada y eficaz, se diseñó un programa de sesiones presenciales combinadas con telerrehabilitación (Tele-RHB) utilizando el entrenamiento muscular del suelo pélvico (EMSP) como tratamiento de primera lÃnea y evaluando la adherencia como factor clave.

Introducción
Durante la pandemia por COVID-19 se vieron afectados los servicios de rehabilitación no esenciales, entre ellos los de incontinencia urinaria (IU).
La IU puede afectar a una de cada cuatro personas, asociarse con repercusiones negativas en calidad de vida y para el sistema sanitario.
Con el fin de mantener una atención continuada y eficaz, se diseñó un programa de sesiones presenciales combinadas con telerrehabilitación (Tele-RHB) utilizando el entrenamiento muscular del suelo pélvico (EMSP) como tratamiento de primera línea y evaluando la adherencia como factor clave.
Objetivos
Principal: Valorar si existe mejoría de la IU al realizar un programa de rehabilitación combinado.
Secundario: Analizar el grado de satisfacción y adherencia al tratamiento.
Metodología
Estudio descriptivo, prospectivo, en el que se evaluaron y trataron 29 pacientes, de ambos sexos, con edades comprendidas entre 31-86 años, diagnosticados de IU que efectuaron el programa durante marzo 2020 y noviembre 2021.
La intervención consistió en añadir a las sesiones presenciales documentos de ejercicios abdómino-pélvicos con códigos QR enlazados a vídeos en internet y continuar la progresión mediante sesiones por videollamada (o llamada telefónica).
Se utilizó el programa estadísticos IBM SPSS para el análisis de las variables principales: ICIQ-SF, Sandvik, protectores, satisfacción y adherencia. El estudio fue diseñado teniendo en cuenta el respeto integral del paciente.
Resultados
La media de edad fue de 62 años. 55% mujeres y 45% hombres.
La mayoría realizaron 4 sesiones presenciales y 4 virtuales (86,2% videollamadas).
Se observaron diferencias significativas en las puntuaciones medias de ambos cuestionarios ICIQ-SF inicial 12,83 y final 9,62 (P=0,000476); Sandvik inicial 6,62 y final 3,79 (P=0,000071).
La proporción de pacientes que inició tratamiento utilizando protectores 86,2% disminuyó significativamente tras finalizarlo 58,6% (P=0,008). La satisfacción media fue 18,24 de un máximo de 20 puntos.
El porcentaje de adherencia al EMSP ≥3 veces/semana 65,5%, 2 veces/semana 31% y no adheridos 3,4%.
Discusión
Los hallazgos publicados actualmente demuestran la eficacia de la telemedicina en mujeres con IU (1-5). La presente investigación fue novedosa al combinar tratamiento presencial con Tele-RHB (sincrónica y asincrónica) para la rehabilitación de IU en adultos de ambos sexos.
Estos datos podrían resultar interesantes por su aplicabilidad y sembrar bases de futuras investigaciones.
Las limitaciones fueron no utilizar reclutamiento probabilístico; grupo control y cegamiento, instrumentos de medida objetivables, ni monitorizar adherencia domiciliaria.
Conclusiones
Tras analizar los resultados se observaron diferencias significativas a nivel cuantitativo (cuestionarios de severidad de IU y protectores), además de obtener puntuaciones elevadas en las variables de satisfacción y adherencia.

Introduction

Main: Assess if there is an improvement in the UI when carrying out a combined rehabilitation program.
Secondary: Analyze the degree of satisfaction and adherence to treatment.
Methodology
A descriptive and prospective study, in which 29 patients of both sexes, aged between 31-86 years, diagnosed with UI who underwent the program between March 2020 and November 2021, were evaluated and treated.
The intervention consisted of adding abdomino-pelvic exercise documents with QR codes linked to videos on the Internet to the face-to-face sessions and continuing with the progression through sessions by video call (or telephone call).
The IBM SPSS statistical program was used to analyse the main variables: ICIQ-SF, Sandvik, protectors, satisfaction and adherence.
The study was designed considering the integral respect of the patient.
Results
The mean age was 62 years. 55% women and 45% men.
Most of them held 4 face-to-face sessions and 4 virtual ones (86.2% video calls).
Significant differences were observed in the mean scores of both ICIQ-SF questionnaires, initial 12.83 and final 9.62 (P=0.000476); Initial Sandvik 6.62 and final 3.79 (P=0.000071).
The proportion of patients who started treatment using protectors 86.2% decreased significantly after finishing it 58.6% (P=0.008).
The average satisfaction was 18.24 out of a maximum of 20 points.
The percentage of adherence to the PFMT ≥3 times/week 65.5%, 2 times/week 31% and non-adherent 3.4%.
Discussion
Current published findings demonstrate the efficacy of telemedicine in women with UI (1-5). The present investigation was novel by combining face-to-face treatment with Tele-RHB (synchronous and asynchronous) for the rehabilitation of UI in adults of both sexes.
These data could be interesting due to their applicability and the possibility of leading to future research.
Limitations of the study were: not using probabilistic recruitment; control group and blinding, objective measurement instruments, or monitoring home adherence.
Conclusions
After analyzing the results, significant differences were observed at a quantitative level (UI severity questionnaires and protectors). In addition, high scores in the satisfaction and adherence variables were obtained.
