Abstract: In our area the main cause of lymphedema is secondary, linked to oncological surgery, which associates nodal emptying and radiotherapy of the region. The treatment of choice for lymphedema is complex decongestive therapy (CDT): lymphatic drainage, multilayer bandage, physical exercise and maintenance with custom compression piece. Liposuction is proposed as a technique of choice in chronic lymphedema with fibrosis with little response to CDT.

Introducción
En nuestra área la causa principal de linfedema es la secundaria, vinculada a cirugía oncológica, que asocia vaciamiento ganglionar i radioterapia de la región. El tratamiento de elección de linfedema es la terapia descongestiva compleja (TDC): drenaje linfático, vendaje multicapa, ejercicio físico y mantenimiento con pieza de compresión a medida. La liposucción se plantea como técnica de elección en linfedema crónico con fibrosis con poca respuesta a la TDC.
Objectivos
Nuestro objetivo ha sido mantener la reducción del volumen de la extremidad superior derecha obtenida con la técnica quirúrgica de la liposucción y dar a la paciente las herramientas necesarias para el autocuidado.
Material y métodos
Caso clínico de una mujer de 54 años afectada de linfedema crónico de la extremidad superior derecha secundario a cáncer de mama. Se le practicó una liposucción por empeoramiento del dolor y pesadez de la extremidad.
Previamente a la cirugía se realizaron quince sesiones de TDC. Dos días después de la cirugía inició TDC, con sesiones de una hora cinco días a la semana con un total de veintinueve sesiones. Las sesiones consistieron en drenaje linfático manual, vendaje multicapa de la extremidad y educación sanitaria con recomendaciones a nivel de cuidado e higiene, posturas que favorezcan el drenaje y ejercicios activos. Al inicio, mitad y al final de las sesiones se utilizó la medición perimetral para valorar la evolución de linfedema. A mitad de las sesiones se prescribió una manga compresiva con guante a medida que la paciente utiliza a diario. Durante la noche usa una manga autofit mobiderm.
Resultados
El resultado ha sido un mantenimiento en el tiempo de la reducción de linfedema obtenida con la cirugía. La paciente ha conseguido una mejor calidad de vida y ha reducido el dolor y el peso de su extremidad.
Conclusiones
Según la literatura es importante después de una liposucción por linfedema crónico realizar un tratamiento de TDC. La confección de una manga de compresión de tejido plano es importante para el mantenimiento de la mejora obtenida y evitar recidivas. La implicación de la paciente para la realización de los ejercicios activos y la correcta colocación de la manga compresiva ha sido una parte fundamental del tratamiento.

Objectives
Our goal has been to maintain the reduction of the volume of the right upper extremity obtained with the surgical technique of liposuction and give the patient the necessary tools for self-care.
Methodology
Clinical case of a 54-year-old woman affected by chronic lymphedema of the right upper extremity secondary to breast cancer. She underwent liposuction for worsening pain and heaviness of the limb. Before surgery, five sessions of CDT were performed. Two days after surgery he started BDD, with one-hour sessions, five days a week with a total of twenty-nine sessions. The sessions consisted of manual lymphatic drainage, multilayer bandage of the limb and health education with recommendations of care and hygiene, postures that favor drainage and active exercises. At the beginning, middle and end of the sessions, perimeter measurement was used to assess the evolution of lymphedema. Halfway through the sessions, a compressive sleeve with a glove was prescribed as the patient wears it daily. During the night she wears an autofit mobiderm sleeve.
Results
The result has been a maintenance over time of the reduction of lymphedema obtained with surgery. The patient has achieved a better quality of life and has reduced the pain and weight of her limb.
Conclusion
According to the literature it is important after liposuction for chronic lymphedema to perform a CDT treatment. The preparation of a flat tissue compression sleeve is important for the maintenance of the improvement obtained and avoid recurrences. The involvement of the patient for the performance of the active exercises and the correct placement of the compressive sleeve has been a fundamental part of the treatment to maintain the results.
