Abstract: Although in vivo slides are being investigated for two decades by high-frequency ultrasound scans and subsequent calculations, the authors who investigate the distal longitudinal sliding of the sciatic nerve in hamstring level (DLSSNHL) have not focused on studying this slide according to the neurodynamic mobilizations (NDM) of the main nerve branches of the sciatic nerve (tibial nerve, superficial peroneal nerve, deep peroneal nerve, sural nerve). Objective: The aim is to determine which NMD (active or passive) causes a higher DLSSNHL in asymptomatic college students.

Introducción
Aunque llevas dos décadas investigando sobre los deslizamientos in vivo gracias a la ecografía de alta frecuencia y los posteriores cálculos correspondientes, los autores que investigan el deslizamiento longitudinal distal del nervio ciático a nivel isquiosural (DLDNCNI) no se han centrado en estudiar este deslizamiento según las movilizaciones neurodinámicas (MND) de las ramas principales del nervio ciático (nervio tibial, nervio peroneal superficial, nervio peroneal profundo, nervio sural).
Objetivo
El principal objetivo es determinar qué MND (activa o pasiva) provoca un mayor DLDNCNI en estudiantes universitarios asintomáticos.
Metodología
Este estudio observacional analítico se realizó en la Clínica Universitaria de la Universidad de Manresa con una duración de 17 semanas.
Las personas participantes se colocaron en decúbito lateral izquierdo con 60° de flexión de cadera y la rodilla con extensión completa, y realizaron de forma aleatoria las MND de forma activa o pasiva (con la ayuda de un fisioterapeuta) modificando la posición del tobillo según la rama a valorar.
Simultáneamente, el DLDNCNI se registró con un ecógrafo de alta frecuencia, y posteriormente, se calculó con el programa informático MatLab software version 2019b a través del "frame by frame cross correlation system".
La variable independiente son las MND (activas o pasivas) y la variable dependiente es el DLDNCNI. Se analizaron las variables con el programa informático Stata 17 mediante la prueba U de Mann-Whitney.
Resultados
22 sujetos participaron de forma voluntaria en el estudio (17 mujeres, 5 hombres) con una edad media de 26,36 ± 5,9 años y un Índice de Masa Corporal (IMC) de 22,09 ± 2,6 kg/m2. El DLDNCNI de forma activa fue de 1,81 ± 0,85 mm, 1,27 ± 0,69 mm, 1,22 ± 0,60 mm y 1,49 ± 0,83 mm por los nervios tibial (NT), peroneal superficial (NPS), peroneal profundo (NPP), y sural (NS), respectivamente. De forma pasiva, el DLDNCNI fue de 1,17 ± 0,81 mm, 1,21 ± 0,71 mm, 0,69 ± 0,44 mm y 1,20 ± 0,83 mm, respectivamente. Existe una diferencia significativa entre el movimiento activo y pasivo del NT (p=0,0013) y el nervio NPP (p=0,0005) y no significativa por el NPS (p=0,7175) y el NS (p=0 ,2565).
Discusión
Varios estudios han investigado cuantitativamente los deslizamientos neurodinámicos descritos teóricamente en los libros, y algunas movilizaciones parecen no seguir del todo estas teorías.
La cantidad de deslizamiento longitudinal del nervio ciático depende de las articulaciones implicadas en el movimiento y del tipo de movilización neurodinámica, obteniendo valores similares en este estudio, así como valores distantes (de 6,2 mm en movimientos de extensión de rodilla hasta valores de 12,4 mm, dependiendo de la posición inicial realizada).
En la extremidad superior, se ha investigado el deslizamiento longitudinal del nervio radial con movimientos activos y pasivos de muñeca, y no se ha obtenido diferencia significativa entre ellos. En cambio, en este estudio se ha alcanzado diferencia significativa entre el movimiento activo y pasivo de tobillo por el DLDNCNI en el nervio tibial y peroneal profundo, siendo el activo el que provoca mayor deslizamiento distal.
Conclusión
El nervio ciático puede movilizarse a partir de sus ramas terminales tanto de forma pasiva como activa, donde esta última es más recomendable.

Introduction

Methodology
This analytical observational study was carried out at the University Clinic of the University of Manresa with a duration of 17 weeks.
Participants were placed in left lateral decubitus with 60° hip flexion and knee fully extended, and randomly performed NDMs actively or passively (with a physiotherapist) modifying the position of the ankle according to the branch to be assessed.
Simultaneously, the DLSSNHL was recorded with a high frequency ultrasound, and subsequently calculated with the computer program MatLab software version 2019b using the "frame by frame cross correlation system".
The independent variable is the NDMs (active or passive) and the dependent variable is the DLSSNHL. Variables were analyzed with the Stata 17 software using the Mann-Whitney U test.
Results
22 subjects participated voluntarily in the study (17 women, 5 men) with a mean age of 26.36 ± 5.9 years and a Body Mass Index (BMI) of 22.09 ± 2,6 kg /m2. The active DLSSNHL was 1.81 ± 0.85 mm, 1.27 ± 0.69 mm, 1.22 ± 0.60 mm, and 1.49 ± 0.83 mm for the tibial (TN) nerve, superficial peroneal (SPN), deep peroneal (DPN), and sural (SN), respectively. Passively, the DLSSNHL was 1.17 ± 0.81 mm,
1.21 ± 0.71 mm, 0.69 ± 0.44 mm, and 1.20 ± 0.83 mm, respectively. There is a significant difference between the active and passive movement of the TN (p = 0.0013) and the DPS nerve (p = 0.0005) and not significant for the SPN (p = 0.7175) and the SN (p = 0,2565).
Discussion
Several studies have quantitatively investigated the neurodynamic sliding theoretically described in books, and some mobilizations do not seem to fully follow these theories.
The amount of longitudinal sliding of the sciatic nerve depends on the joints involved in the movement and the type of neurodynamic mobilization, obtaining similar values in this study, as well as distant values (from 6.2 mm in knee extension movements to values of 12.4 mm, depending on the initial position performed).
In the upper extremity, the longitudinal sliding of the radial nerve with active and passive wrist movements has been investigated, and there has been no significant difference between them. In contrast, this study found a significant difference between the active and passive movement of the ankle by the DLSSNHL in the tibial nerve and deep peroneal nerve, the active being the one that causes greater distal sliding.
Conclusion
The sciatic nerve can be mobilized from its terminal branches both passively and actively, where the active is more recommended.
