Efficacy of the physiotherapeutic manual approach to gastroesophageal reflux disease in patients with type I hiatus hernia

Abstract: Gastroesophageal reflux disease (GERD) is highly prevalent in society. Hiatus hernia (HH) is etiologically determining in the appearance or aggravation of the symptoms of these patients. Many patients will permanently follow pharmacological treatment with proton pump inhibitors (PPIs) or H2 blockers. The surgical option is reserved for those patients who do not respond to conservative treatment or with contraindications to it. Physiotherapy proposed respiratory and diaphragmatic training to improve the effectiveness of anti-reflux barriers, with optimal results. Other studies have evaluated the efficacy of manual therapy on the thoracic diaphragm or osteopathic techniques on the cervical and thoracic region, with good results.
Abstract : La enfermedad por reflujo gastroesofágico (ERGE) es altamente prevalente en la sociedad. La hernia de hiato (HH) es etiológicamente determinante en la aparición o agravamiento de la clínica de estos pacientes. Muchos pacientes seguirán de forma permanente un tratamiento farmacológico con inhibidores de la bomba de protones (IBPs) o bloqueadores de H2. La opción quirúrgica se reserva para aquellos pacientes que no responden al tratamiento conservador o con contraindicaciones al mismo. La fisioterapia propuso el entrenamiento respiratorio y diafragmático para mejorar la eficacia de las barreras antirreflujo, con resultados óptimos. Otros estudios han evaluado la eficacia de la terapia manual sobre el diafragma torácico o las maniobras osteopáticas sobre la región cervical y torácica, con buenos resultados.Gastroesophageal reflux disease (GERD) is highly prevalent in society. Introducción
La enfermedad por reflujo gastroesofágico (ERGE) es altamente prevalente en la sociedad. La hernia de hiato (HH) es etiológicamente determinante en la aparición o agravamiento de la clínica de estos pacientes. Muchos pacientes seguirán de forma permanente un tratamiento farmacológico con inhibidores de la bomba de protones (IBPs) o bloqueadores de H2. La opción quirúrgica se reserva para aquellos pacientes que no responden al tratamiento conservador o con contraindicaciones al mismo. La fisioterapia propuso el entrenamiento respiratorio y diafragmático para mejorar la eficacia de las barreras antirreflujo, con resultados óptimos. Otros estudios han evaluado la eficacia de la terapia manual sobre el diafragma torácico o las maniobras osteopáticas sobre la región cervical y torácica, con buenos resultados.
Objetivo
Evaluar el efecto de un protocolo de terapia manual en el impacto de la ERGE por HH tipo I.
Material y métodos
Ensayo clínico a doble ciego, con grupo intervención y grupo control. El grupo intervención recibe tres sesiones de tratamiento para la ERGE y HH. El grupo control recibe un tratamiento placebo. Se utiliza el test de GIS y una escala EVA con 5 ítems para evaluar el impacto de la ERGE de los pacientes.
Resultados
El test de GIS muestra una mejora clínica en los pacientes del grupo intervención una semana después de la finalización del tratamiento aumentando una media de 11,59 puntos (DesvEst: 5,43) (IC95%: -14,003 - -9,179; p-valor=0,000). La escala EVA utilizada por el servicio de Cirugía Digestiva también muestra una mejora clínica en este grupo con una disminución de media de 18,86 puntos (DesvEst: 9,40) (IC95%: 14,695 – 23,033; p-valor=0,000). Los resultados en el grupo intervención se mantienen a medio plazo. No se aprecia mejoría en el grupo control.
Conclusiones
El abordaje manual de la ERGE por HH tipo I, resulta efectivo al mejorar el impacto de la enfermedad a corto y medio plazo.

Objective
To evaluate the effect of a manual therapy protocol on the impact of GERD due to HH type I.
Material and methods
A double-blind clinical trial, with intervention group and control group. The intervention group receives three treatment sessions for GERD and HH. The control group receives a placebo treatment. The GIS test and a 5-item VAS scale are used to assess the impact of GERD on patients.
Results
The GIS test shows a clinical improvement in the patients in the intervention group one week after the end of the treatment, increasing an average of 11.59 points (SD: 5.43) (95% CI: -14.003 - -9.179; p -value=0.000). The VAS scale used by the Digestive Surgery service also shows a clinical improvement in this group with a mean decrease of 18.86 points (SD: 9.40) (95% CI: 14.695 – 23.033; p-value=0.000). The results in the intervention group are maintained in the medium term. No improvement is seen in the control group.
Conclusions
The manual approach to GERD due to HH type I is effective in improving the impact of the disease in the short and medium-term.
