Abstract : El ingreso en la unidad de cuidados intensivos (UCI), la ventilación mecánica (VM), la sedación y la propia patologÃa crÃtica generan un deterioro fÃsico evidente y la aparición de debilidad muscular adquirida en los pacientes. La movilización precoz (MP) son un conjunto de intervenciones y tratamientos planificados con una progresión determinada para mantener y reestablecer la capacidad funcional y la autonomÃa del paciente crÃtico.

Introducción
El ingreso en la unidad de cuidados intensivos (UCI), la ventilación mecánica (VM), la sedación y la propia patología crítica generan un deterioro físico evidente y la aparición de debilidad muscular adquirida en los pacientes. La movilización precoz (MP) son un conjunto de intervenciones y tratamientos planificados con una progresión determinada para mantener y reestablecer la capacidad funcional y la autonomía del paciente crítico.
Objetivos
El objetivo principal es describir la evolución del ejercicio realizado por los fisioterapeutas desde la implantación del protocolo de MP y valorar el impacto de este en la recuperación física de los pacientes.
Material y métodos
Estudio descriptivo y retrospectivo (2017-2021) del desarrollo del protocolo de MP en una UCI polivalente de un hospital de tercer nivel. Se recopilaron datos epidemiológicos, de actividad y de evolución. Datos expresados en frecuencias, porcentajes, índices de tendencia central y de dispersión según su distribución. SPSSv.21.
Resultados
Se incluyeron un total de 1349 pacientes (65,8 hombres, edad media 60,4±14.65, 81,8% con un diagnóstico médico de ingreso, APACHE II 20,7±7,5). Los pacientes presentaron una estancia media de 13.9 (1; 193) días e iniciaron el protocolo de MP el día 6,34±4,9, recibiendo una media de 7,6±4,2 sesiones de fisioterapia durante el ingreso. Sólo en el 4,2% de las sesiones se produjeron eventos adversos. Al alta de la UCI, el 23,7% de los pacientes consiguieron el nivel de protocolo 4 y el 23,3% el nivel 5, objetivándose así un aumento significativo entre el primero y el último MRC (p<0.001). En el 42% de los pacientes se encontró como mínimo un registro de "Falta de personal".
Discusión
En relación a la mejora del paciente al alta de la UCI, diferentes estudios confirman que los protocolos de MP reducen el tiempo de recuperación del paciente; otros evidencian la disminución de los días de VM así como la mejoría funcional global. En términos de seguridad, la bibliografía afirma que la MP es a términos generales segura y factible. Por último, es importante destacar la importancia de la implementación de estos protocolos de manera precoz, ya que según algunas publicaciones, este hecho tiene un efecto preventivo en la aparición de la debilidad muscular del paciente crítico.
Conclusiones
Los pacientes críticos incluidos en el protocolo de MP logran la bipedestación y/o la deambulación cuando son dados de alta a la planta de hospitalización. La presencia de fisioterapeutas en la UCI es fundamental para una óptima recuperación física de los pacientes.

Introduction
Admission to the intensive care unit (ICU), mechanical ventilation (MV), sedation and the critical pathology itself generate obvious physical deterioration and the appearance of acquired muscle weakness in patients. Early mobilization (EM) is a set of interventions and treatments planned with a certain progression to maintain and re-establish the functional capacity and autonomy of the critically ill patient.
Aim of the study
The main objective is to describe the evolution of the exercise performed by physiotherapists since the implementation of the EM protocol and to assess its impact on the physical recovery of patients.Materials and methods
Descriptive and retrospective study (2017-2021) of the development of the EM protocol in a polyvalent ICU of a tertiary care hospital. Epidemiological, activity and evolution data were collected. Data expressed in frequencies, percentages, indices of central tendency and dispersion according to their distribution. SPSSv.21.Results
A total of 1349 patients (65.8 men, mean age 60.4±14.65, 81.8% with a medical diagnosis on admission, APACHE II 20.7±7.5) were included. The patients had a mean stay of 13.9 (1; 193) days and started the EM protocol on day 6.34±4.9, receiving a mean of 7.6±4.2 physiotherapy sessions during admission. Adverse events occurred in only 4.2% of the sessions. Upon discharge from the ICU, 23.7% of patients achieved protocol level 4 and 23.3% level 5, thus showing a significant increase between the first and last MRC (p<0.001). In 42% of the patients, at least one record of "Lack of staff" was found.DiscussionIn relation to patient improvement upon discharge from the ICU, different studies confirm that EM protocols reduce patient recovery time; others show a decrease in the days of MV as well as an overall functional improvement. In terms of safety, the literature states that EM is generally safe and feasible. Finally, it is important to highlight the importance of early implementation of these protocols, since according to some publications, this fact has a preventive effect on the appearance of muscle weakness in critically ill patients.Conclusions
Critically ill patients included in the EM protocol achieve standing and/or ambulation when they are discharged to the hospitalization ward. The presence of physiotherapists in the ICU is essential for optimal physical recovery of patients.Bibliography
