Abstract: Tendinopathy remains one of the most prevalent musculoskeletal pathologies and, at the same time, one of the most clinically heterogeneous. Despite the growing volume of research and the existence of multiple therapeutic options, the available evidence shows variable and, in many cases, limited results for interventions such as corticosteroid injections, platelet-rich plasma, shockwave therapy, or surgery. Meanwhile, therapeutic exercise remains the gold standard treatment, although uncertainties persist regarding the mechanisms that explain its effects and the optimal strategy for each patient.
Abstract : La tendinopatía continúa siendo una de las patologías musculoesqueléticas más prevalentes y, al mismo tiempo, una de las más heterogéneas desde el punto de vista clínico. A pesar del creciente volumen de investigación y de la existencia de múltiples opciones terapéuticas, la evidencia disponible muestra resultados variables y, en muchos casos, limitados para intervenciones como las infiltraciones de corticoides, el plasma rico en plaquetas, las ondas de choque o la cirugía. Paralelamente, el ejercicio terapéutico se mantiene como el tratamiento de referencia, aunque todavía persisten incertidumbres sobre los mecanismos que explican sus efectos y sobre cuál es la estrategia óptima para cada paciente.
