    Abstract: Pain-associated temporomandibular disorders (TMD) are the leading cause of chronic orofacial pain of musculoskeletal origin, with a prevalence of up to 15%. Biobehavioral approaches are crucial for managing chronic pain.

    Introducción

    Los trastornos temporomandibulares (TTM) asociados a dolor son la primera causa de dolor orofacial crónico de origen músculo esquelético con una prevalencia de hasta el 15%. Los enfoques bio conductuales son cruciales para el manejo del dolor crónico.

    Objetivos

    Repasar la fisiopatología de los TTM crónicos desde el punto de vista biopsicosocial. Analizaremos la evidencia actual respecto a los procesos neurobiológicos (dolor nociceptivo, sensibilización central, sensibilidad central, dolor nociplástico y las condiciones de dolor crónico comórbido) y su correlación con factores psico sociales (kinesofobia, catastrofismo, discapacidad). Además de ofrecer unas guías clínicas basadas en la literatura científica existente.

    Introduction

    Pain-associated temporomandibular disorders (TMD) are the leading cause of chronic orofacial pain of musculoskeletal origin, with a prevalence of up to 15%. Biobehavioral approaches are crucial for managing chronic pain.

    Objectives

    To review the pathophysiology of chronic TMD from a biopsychosocial perspective. We will analyze current evidence on neurobiological processes (nociceptive pain, central sensitization, nociplastic pain, and comorbid chronic pain conditions) and their correlation with psychosocial factors (kinesiophobia, catastrophizing, disability). Additionally, we will provide clinical guidelines based on existing scientific literature.
