Abstract: Left main coronary artery (LMCA) atherosclerotic obstructive disease represents a form of high-risk coronary artery disease since the myocardial area at risk is very extensive, accounting for approximately 70% of the left ventricular mass. Initially, coronary bypass surgery (CABG) was the only myocardial revascularization treatment for this type of complex coronary anatomy. Although surgery has important advantages, the idea of being able to offer minimally invasive treatments has led to percutaneous coronary intervention (PCI) with stent implantation as an alternative. In this review, we will discuss in detail all the randomized clinical trials that have compared these two interventions and will put their results in perspective.
